Take the stress out of setting up an appointment. Here are some helpful to prepare you for your first appointment.
When Should I Arrive?
Please arrive 10-15 minutes before your scheduled appointment time. This will allow adequate time to complete all the necessary paperwork. If possible, please bring the complete forms ahead of time so we can get you started even sooner.

What Should I Wear and Bring?
Please wear comfortable, flexible clothing to your appointments. We ask that you try not to wear any jewelry. At your first appointment, please bring:

Photo identification (valid driver’s license, valid state ID, school ID card)

Your physical therapy prescription/doctor referral (if applicable)

Insurance card


What Will My First Appointment Look Like?
After submitting your completed intake paperwork, our front office staff will verify all your information.
You will meet with one of our licensed physical therapists to review your medical history, current pain and other recent events that apply. We encourage open communication for your best results!
Your evaluation will also include an assessment of your current movement, strength, balance, coordination, functional deficits, pain level, posture, and more depending on your condition. Our end goal is to help you, so please ensure you communicate to your therapist if anything is hurting or bothering you during the examination.
Your therapist will decide on an appropriate treatment plan based on your current ability or prescription. This plan can be shared with your referring physician or other medical professional who you choose.
After your initial appointment, check in with the front desk staff to get an overview of what your future appointments will occur.
To give you the best possible care, we encourage you to communicate with our team whenever you have a question during any step of the treatment process. We want you to always feel like you are an active participant in your recovery.
